Emergency medicine shows what fails in the human body and how late problems are caught. Medical toxicology adds a mechanistic understanding of how medications and substances behave in each individual. Longevity medicine applies both disciplines decades earlier, while there is still time to change the trajectory.

FAQ

The details, plainly.

Can Perennial be my primary care physician?

Yes, for generally healthy adults with common, stable needs — for example, someone managing blood pressure or cholesterol who does not need ongoing specialist-led care. People with complex or specialist-managed conditions keep their existing care team, and Perennial coordinates alongside it.

Do you accept insurance?

No. Perennial does not bill insurance for the assessment or membership. Labs, imaging, and other outside services are separate; depending on the provider and your benefits, those services may be billed to your insurance.

What happens after testing?

Testing is interpreted in context, translated into priorities, and revisited over time. The goal is not more data; it is better clinical decisions.

How do patients communicate?

Patients use the secure Perennial portal for labs, documents, plan tracking, visits, and communication. Concierge care includes a higher level of direct access.

Can I use HSA or FSA funds?

Some patients may be able to use HSA or FSA funds for eligible medical services. Perennial provides transparent receipts, but patients should confirm eligibility with their plan administrator or tax advisor.

What is included in membership?

Membership includes ongoing physician-led care, structured follow-up, secure portal communication, plan review, and clinical interpretation of selected diagnostics. Outside labs, imaging, or third-party testing may be billed separately.

What if I need urgent or emergency care?

Perennial supports longitudinal longevity care and concierge access, but emergencies still require 911 or emergency department care. Urgent-care expectations and scope of care are reviewed with members during onboarding.

Can I cancel membership?

The Baseline Assessment never obligates you to enroll. Memberships are billed monthly; the written agreement you receive before enrollment states the cancellation and renewal terms.

Does Perennial provide hormone therapy?

Hormones are evaluated as part of the full clinical picture, not treated as a default longevity product. When symptoms, history, examination, and testing support treatment, Dr. Mottram discusses evidence-based options, expected benefits, risks, and appropriate monitoring.

Can I bring my existing wearable and lab data?

Yes, and please do. Your prior labs, wearable history, and imaging are reviewed before your first visit and brought into Perennial OS.
